would I know that I made these people lonely ugly, and sad? what if I make friends with those people? and use money made form being an actor or model (or whatever) to help them with surgeries, and thereapy for their loneliness, and sadness...

The two are not connected. The ten sad ugly people will be sad and ugly even if the button is never pushed by anyone . Consquently , pushing the button won't cause ten people to become sad and ugly. I'm still not pressing the button
