One day... Ok, just do the wish at 12:59:58 PM. The pain will be 2-seconds-long as if it were to go unto the next day, it would be 2 days instead of 1. Be sure sure your clock is perfectly sincronized.

I assume it"s a slow and progressive change that explains the extreme pain involved with the Downside. I don't care about that, though, as this is practically a WIN-WIN. Painkillers are an easy loop-hole. Also, a progressive change prevents shock, I guess.
