The downside is always the concept of immortality itself. Even if seeing everyone you care about die isn't bad to you, you have eternity. Eternity is torture.

So, what i think this question implies is that you can still die in like a car crash, because it doesn't say that I'm immortal, i just COULD live forever if i wanna, but when i get bored i can just💀
